Form I (A) - for Senior Grade Assistants/Selection Grade Assistants and other Non-Gazetted staff.
1. Performance Appraisal Form I (A) is meant for the category of Senior Grade Assistants in the Secretariat. The same will also be used for Legal Assistant Grade I in the Law Department and Reception Officers of the Secretariat. It is also meant for Typist/Stenographers and Office Superintendents who come out successful in the suitability test conducted by the Public Service Commission for appointment as Section Officers from the date on which they are posted for training as Assistants.
2. The form will also be used for the personnel belonging to the above said categories working in other Departments/ Instructions on deputation, Foreign Service conditions etc.
3. The form consists of two parts viz. Part I and Part II. The facing sheet of Part I will be got filled up by the reportee. A note on "Self Assessment" should be obtained from the reportee and filed as part of the facing sheets. "Self Assessment" in this context means a brief resume of work done by the Officer reported upon during the period under report bringing out any special achievement of his/her during the period, to be filled by the Officer reported upon. The resume should not exceed three hundred words. The "Self Assessment" given by the reportee will help the Reporting Officer to make the ratings. The graphic portion of Section-A of Part I will be prepared by making the appropriate grading items against the factors given. Part I will be shown to the reportee and his signature obtained. Part II is meant for recording the assessment of the Reporting/ Reviewing Officer on the loyalty and integrity of the reportee. This part will be considered as 'secret' and should not be shown to the reportee. This will not be a basis for determining the officer's suitability for promotion/appointment to higher posts.
4. Special care should be taken in filling up the performance appraisal forms by the Reporting and Reviewing Officers.
5. The rating should be done taking into account the individual's actual performance on the job during the period under report.
6. The Reporting Officer should take special care to study the factors for appraisal and apply them judiciously on the reportee's performance. Normally, an Impartial Officer can given only a few number of 'A' Grades to an Officer reported upon. Award of 'A' Grades should be substantiated.
7. The Reviewing Officer will record his remarks in narrative form in the column provided in the forms. If he finds its necessary to revise the gradings given by the Reporting Officer he will do so, in which case the gradings given by the former will prevail.
8. It is the duty of the Reporting Officers/Reviewing Officers to see that no item in the form is left unfilled.
9. In the case of Senior Grade Assistant the Reporting Officer will write up the form in consultation with the Section Officer under whom the reportee is working.
10. The Reporting Officer is respect of a person in the category of Senior Grade Assistant shall be an Officer not below the rank of an Under Secretary under whom the person is working and the Reviewing Officer shall be the authority to which the Reporting Officer is subordinate. In all case, the Officer to whom one reports shall write up the performance appraisal forms irrespective of the rank of the Reportee.
11. In all other matters viz. disposal of representations received from the reportees, period of writing the report, etc. and on matters not covered by these instructions, the procedure laid down in G.O.(P) No. 344/PD dated 22-8-1966 as amended from time to time will apply.
Form I (B) - for Section Officers and Under Secretaries
This from is also used for the Accounts officers in the Secretariat and employees belonging to all these categories working in other Departments/Institutions on deputation.
1. Performance Appraisal Form 1 (B) is meant for the category of Section officers and Under Secretaries in the secretariat.It will also be used for the categories like Chief Reception Officers and Accounts Officers(CPPF) in the Secretariat
2. The form will also be used for the personnel belonging to the above said categories working in other Departments/ Instructions on deputation, Foreign Service conditions etc.
3. The form consists of two parts viz. Part I and Part II. The facing sheet of Part I will be got filled up by the reportee. A note on "Self Assessment" should be obtained from the reportee and filed as part of the facing sheets. If the reportee does not submit the Performance Appraisal Form to the Reporting Officer with the facing sheet of Part I filled in together within 15 days after the report has become due,the Reporting Officers will arrange to have the Performance Appraisal Form filled up from the records available,will record their remarks and forward the same to the Reviewing officer.It is left open to the reportee to present the self assessment to the Reviewing Authority bofore the Reviewing Authority records his remarks in the Perfomance Appraisal of the officer.In that case,the Reviewing Officer will take into account the self assessment also before recording his remarks.In the case of part reports,consequent on change of Reporting Officer,reportee's proceeding on leave and rejoining under another Reporting Officer,transfer etc.,the same time limit of 15 days will apply from the date of such change,rejoining,transfer etc., for presenting the 'self Assessment' by the reportee to the reporing Officer.

11. In the case of Under Secretary working under more than one Deputy secretary/Joint Secretary/Secretary/Minister as the case may be,the Performance Appraisal Form will be written by mutual consultation.
12. In all other matters viz. disposal of representations received from the reportees,period of writing the report,etc. and on matters not covered by these instructions,the procedure laid down in G.O.(P) No.344/PD dated 22-8-1966 as amended from time to time will apply.
Form I (C) - for Deputy Secretaries, Joint Secretaries and Additional Secretaries in Secretariat. (Non IAS Officers), including employees belonging to all these categories working in other Departments/Institutions on deputation.
1. Performance Appraisal Form I(C) is meant for Deputy Secretaries,Joint Secretaries and Additional Secretaries in the Secretariat.(Other than those belonging to the All india Services)
2. The form will also be used for the personnel belonging to the above said categories working in other Departments/ Instructions on deputation, Foreign Service conditions etc.

7.The performance appraisals of Deputy Secretaries,Joint Secretaries and Additional Secretaries will be placed before the "Moderation Committee" before they are submitted to the Minister concerned.
8. The Reviewing Authority will record his remarks in a narrative form in the column provided in the form.If he finds its necessary to revise the gradings he will do so,in which case the gradings given by the Reviewing Authority will prevail.
9. It is the duty of the Reporting Officers/Reviewing Officers to see that no item in the form is left unfilled.
10. The Reporting Officer in respect of Deputy Secretary/joint Secretary/Additional Secretary to Government will be the Secretary concerned and the Reviewing Authority will be the Minister concerned.In all cases,the Officer to whom one reports shall write up the performance appraisal forms irrespective of the rank of the Reportee.The Reporting Officer in the case of Administrative Officer/Finance Officer etc.,should be the Head of the Department concerned and it will be submitted to the Reviewing Officer through the Secretary/Special Secretary concerned.
11. In the case of Deputy Secretary/Joint Secretary/Additional Secretary working under more than one Secretary/Minister as the case may be the case the Performance Appraisal Form will be written by mutual consultation.
12. In all other matters viz. disposal of representations received from the reportees,period of writing the report,etc. and on matters not covered by these instructions,the procedure laid down in G.O.(P) No.344/PD dated 22-8-1966 as amended from time to time will apply.
